I am a friend or relative of a current/past resident
My husband was admitted there about noon yesterday the 30th and last night I got a call at 1:20 that he was having chest pain and had rang for the nurse for over 40 minutes. so I called I had a hard time getting anyone to answer the phone finally she went and took his vitals, I told her I was going to call the squad she said she didn't think that was necessary said vitals were not that bad he has a very weak heart type two diabetic, skin tears is all over his body , Plus a broken foot, and needs care when his light goes off not 40 minutes later. Then he started bleeding from His skin tears and was soaking it up with his handkerchiefs Again staff did not come to address it for quite a while, he is close to end of life and needs care which I thought he would get there. So this morning my daughter and I went out and signed him out AMA to die at home because he can't get the care there. I would not recommend any of my loved ones to be there again

My mother in law is a current resident of Troy Care and Rehabilitation Center. I didn't know much about the facility, but it was the recommendation from a caseworker when we were in the hospital. She's currently using a bedpan. We were told when they left the hospital that she wasn't rehab material, but the other facility is going to work with her in other ways. She's also not been very happy with the food. They were still giving her ground up food and stuff, and my mother-in-law has been very shy about telling them what she would have liked. That upset me numerous times. The first time I was there to visit her, I had another gentleman that came out and complained about the food, and he actually ordered a lot of food out. I've only been in a room once, and it looked fine. They were going to give me an unremodeled, plain, black, depressing room. The day that I called it off and I told them that I could not possibly put her in that room, they offered me one of the newly remodelled rooms. The facility is an older building, but it's clean. They have air conditioning, but I don't know if they worked since they kind of looked crappy from the outside, but I'd rather focus my attention on the care that they provide. I just didn't want to accept that this is where she's at and this is what she's gonna get because that wouldn't have been this type of care that she would get at home. They needed to improve on their communication too. I know part of that was because she didn't want to speak up, and she was just ready to accept what was coming her way. However, whether the people speak up or not, they still need to have better communication.

The staff have been very patient and caring with my dad. Only issue was lack of communication after he was transferred from Miami Valley and all his medical needs were not addressed in the first few days. It was addresses right away and he is receiving the care he needs.

By Mark's ministerI visited this facility
I was in after normal visiting hours. There was no one at the desk, but one of the residents helped me with the new sign-in device. It is a little cumbersome to sign in, but signing out is a breeze. Mark's room was large and clean. His call button was on when I got there (bathroom). It took a while for anyone to respond. She was very helpful and nice, though. On the next visit, had to wait at the sign-in device for someone having trouble signing in. He never did get it done. When I started signing in, it was evidently still responding to him, so it took a while to get it to reset and sign in. A couple of staff in the hall recognized Mark and called out to him. He seemed to enjoy his interactions with the staff.

Thank you so much for your feedback! While our sign-in kiosk may take some time getting used to, we find that it is ultimately a more secure and safer option in the long run. We are constantly striving to make improvements to our facility and appreciate your thoughts on the matter!

I was there for 4 weeks for rehab therapy, The therapy department is top notch and very caring. The daytime nursing staff is fantastic but at night somewhat unattentive. The facility is very clean. However, the food is terrible and inconsistant. There is just no excuse for the poor quality. They act like they want to improve but the kitchen management is clueless and is full of excuses. The good thing is that I lost 12 pounds. If you like to play bingo a dozen times a week activities are great but other than BINGO there is little choice.

My mother was admitted into this facility for congestive heart failure. She has been there for seven weeks and healthier than she has been in years. The nursing staff is very attentive to her needs, the therapy staff keeps her from giving up. They are a very kind medical staff and my mom doesn't mind being there at all. The facility is clean and orderly. LOVE THIS PLACE!!! I get more time with my mom.

Okay i do want to give a comment. My brother was put in for rehab which was fine. Well we never left him at all. So one morning while looking for a nurse to help my brother to use the urinal no one was in site. So guess what I had to help him my self can you imagine how that had to make him feel. And they bring there drinks about an hour before the meals, which then it was hot or the coffee was cold. Never a nurse in site. He had a few very sweet nurses but cant speak for all of them. Also seen then just setting at the nursing station and it they see the main big chiefs coming the call the other nursing station to let them know they are coming get busy. And as I set with him day in and day out the man across the hall was very neglected they got tired of hearing him call for the nurse they shut his door...I would not remand this place for no one.It stinks. Sorry but its the truth anyways.

I didn't have a bad experience in our visit at Troy Health and Rehab Center. They've got a better staff, and they are more staffed than they are used to be. It was very clean, and in their Alzheimer's unit, they started painting the walls to make it very bright and cheery. They have upgraded their personal rooms, and they have an area where they can go in and out at any time they want to go to the garden area, and there is someone out to be with them. I like that part about it probably the most.

I have a relative in long term care and another one in alzheimer's The biggest issue is the smell. It is a very strong urine smell in the long term care area. The rooms are very small. The 2 paitients in the room can not sit side by side in their wheel chairs. Bathrooms do not accomidate a wheel chair and are very outdated. They seem to be very understaffed and not attentive to the patient needs. The alzheimer's unit has a little bigger rooms, but also has a urine/dirty smell. There have been many times that the patients are sitting in the alzheimer's activity room not doing anything. This center is very run down and dirty. In contrast, the rehab area has been updated and staff is attentive. They need to figure out how to have that through out the center.

During the time my mother-in-law was in the Rehab portion (assisted living portion) of this facility, it was quite good. She had a nice furnished private room, including a television with cable & daily newspaper, and the staff seemed quite responsive to her needs. They provided her meals and rehabilitation for problems she was having walking. We did her laundry and visited almost daily. She seemed quite content there. They had daily activities going on in the activities room and she was able to go down there to play Bingo and do crafts with other residents. They also had musical guests come in and had other performers visit. They also provided church services. They also had a house cat named "Tigger" who roamed the facility and visited her often. This always made her happy. The food was average for a facility, and she seemed to always be able to find something that she liked to eat. There seemed to be a vast difference between this portion of the facility and the nursing home, which is why I rated the facility a "3". I would have probably rated just the Rehab portion a "4" but the nursing home would have only gotten a "2". They were very understaffed and we had to be there almost around the clock to give her the care she needed.

By Dale-1I visited this facility
My grandparents spent years in a home before dying so, unfortunately, I am familiar with nursing homes and how they operate. That said, I was appalled at the lack of bedside manner at this facility. Within the same visit to my husband's grandparents' room, I overheard a nurse across the hall tell her patient that "(he) had just taken the biggest crap of his life, there was no way he had to go again!" and that "this is where you come to either get better or die". NEITHER COMMENTS SEEMED APPROPRIATE TO ME. The nurses who came in to our room to feed our grandparents during our visit were not at all friendly and seemed annoyed and inconvenienced by their jobs. Furthermore, there were no patient charts anywhere to be found in the room to show that the patients were being regularly checked and attended to. The bottle into which Grandpa had to urinate was placed by the nurse on the same table on which he was served his meal -- and neither the bottle nor the roll of toilet paper next to it were removed once his meal was left there. The meal itself was horrific. It was supposed to be turkey pot pie but I saw maybe 2 chunks of what was supposed to be turkey on top of a biscuit, then a mound of frozen peas and carrots beside it. A 4-ounce carton of milk and some coffee were the only beverages provide. No water was given -- which one would think would be important to flushing out the patients' systems with all the drugs they are given. I would never put a loved one in this facility simply because the staff would sink them into depression if their actual physical conditions didn't.
